We know that Tom Paris consistently had a hero collar through the entire show, from when he's first in uniform during "Caretaker" to the end of "Endgame."
Even when he was LARPing in the holodeck, because the VOY undershirt did not change colors for the wearer's division (unlike the DS9/NEM division shirt), he seemed to wear his regular uniform undershirt, as evidenced by both the color and color constructions of his undershirt in Voyager's late third season episode "Worst Case Scenario":

However, for some bizarre reason, in Voyager's late seventh season episode "Author, Author," Tom wore his regular VOY undershirt while on duty but an entirely different VOY undershirt (without a hero collar!) while LARPing:
